To speed things up on the day, please complete a Registration Form and print out both pages separately and bring with you on the day. You will need to bring a Registration Form for each animal you are having microchipped. DO NOT print out on both sides of the paper and DO NOT use staples.

Please Note: The Chipblitz $10 rate is not available to Breeders. Breeder/litter rate is $25 per animal. All those with litters are considered breeders and you will need to have your SA breeder number with you.
